The 12-2-1 Roundtables Intensive Outpatient (IOP) Program provides structured, telehealth-based recovery support for individuals who need more than occasional help, but do not require residential treatment.
Our IOP is designed to help participants move from isolation into connection, from simply “getting through” requirements into meaningful recovery engagement, and from old patterns toward practical steps for lasting change.
Through guided Roundtables, individual support, Challenges, accountability, and honest conversation, participants are given a safe place to reflect, grow, and take their next right step.
A Different Kind of IOP

Our Intensive Outpatient Program is built around the heart of the 12-2-1 Roundtable model:

Who This Program Is For
Our IOP may be helpful for individuals who are:

Working toward recovery from substance use

Stepping down from residential treatment

Involved with Drug Court, Day Report Centers, Probation, Parole, DoHS/CPS, or other referral partners

What Participants Can Expect
Participants in our IOP can expect a welcoming, structured, and recovery-focused environment. Our goal is to create a space where people can be honest without shame, challenged without condemnation, and encouraged without pretending everything is easy.

Why Roundtables Help

A Strong Stepdown from Residential Treatment
For individuals leaving residential treatment, our IOP can serve as an effective stepdown into continued recovery support.
The transition from residential care back into daily life can be challenging. Participants often need structure, connection, accountability, and a place to keep practicing the recovery tools they have started learning.
12-2-1 Roundtables helps bridge that gap by keeping participants engaged in meaningful recovery conversations while they continue building stability, support, and practical skills for life outside of treatment.
Supporting Referral Partners
We are grateful to partner with courts, agencies, treatment centers, DoHS/CPS, Day Report Centers, probation and parole officers, churches, and community organizations.
Our IOP is designed to support both participant growth and referral partner confidence through clear communication, telehealth accessibility, structured programming, and recovery-focused care.
When appropriate, progress updates and communication can be provided through secure systems and referral pathways.
